Default Problem with NEVER_MOUSE in 1DM
The mouse throttle controls are off in 1DM if you have the NEVER_MOUSE option enabled:

Its not just graphical, the turning and throttle controls and offset by some amount. Its only happened to me in 1DM maps so far.

Is that by any chance the New England server?

It (atleast used to be, not sure if it still is) is set to a slight zoom and a slight plane size reduction, maybe it could be that that has to do with the offset?
